About this screener — evidence, source
Measures paranoid thinking over the past month across the spectrum from everyday mistrust to persecutory delusions. Part A (8 items) assesses social ideas of reference; Part B (10 items) assesses ideas of persecution. Published severity ranges let the persecution score be read as average, elevated, moderately severe, severe, or very severe.
Evidence · validated
Derived and validated in 10,551 people, including over 2,000 patients with psychosis (Freeman et al., 2021). Persecution ≥ 11 discriminated patients with persecutory delusions from non-clinical participants with sensitivity 92.8% and specificity 85.2% (AUC 0.95); ≥ 18 had sensitivity 81% and specificity 93% for a likely persecutory delusion. Items were invariant across gender and age groups (13–21 years and older).
