About this screener — evidence, source, alternatives

Nine items scoring each DSM depressive symptom from 0 to 3 over the last two weeks, plus a functional difficulty question. Used for screening, severity grading, and monitoring response to treatment, including in pregnancy and after birth, where it is one of the screens ACOG lists for perinatal depression.

Evidence · validated

At a cut-off of ≥ 10, sensitivity and specificity were both 88% for major depression in the original primary-care validation (Kroenke et al., 2001). Extensively validated across languages and settings, and in adolescents (12+).

Free alternative to

  • Beck Depression Inventory-II (BDI-II) · Pearson
Paid depression severity inventory; the PHQ-9 is the free, criteria-based alternative used in most primary care.
