About this screener — evidence, source, what inspired it

Asks whether there has ever been a distinct period of raised or irritable mood together with increased energy, then which of the seven DSM-5-TR criterion B symptoms were present at the same time, how long the period lasted, whether others noticed, and what it led to (problems, hospital care, psychotic symptoms). The result says whether the reported history fits the pattern of a hypomanic or manic episode. It is most useful before starting an antidepressant and for anyone with depression who has not been asked about mood highs. The validated tools in this area (MDQ, HCL-32) are widely used on paper, but permission for electronic reproduction is required or could not be confirmed, so this original fills the gap. It is not validated.

Evidence · unvalidated

This is an OpenScreener original instrument. Its thresholds are provisional, set by expert judgement rather than population norms, and have not yet been validated. Treat results as a structured record of concerns, not a score with known sensitivity or specificity.

Inspired by

  • Mood Disorder Questionnaire (MDQ) · Hirschfeld et al.; University of Texas Medical Branch
Lifetime yes/no checklist of manic symptoms followed by questions on whether they clustered together and how much trouble they caused.
  • Hypomania Checklist-32 (HCL-32) · Angst et al.
Focus on milder, hypomanic highs that people often recall as positive, and on asking about a specific ‘high’ period.
  • Bipolar Spectrum Diagnostic Scale (BSDS) · Ghaemi, Pies et al.
Attention to the wider bipolar spectrum, including short or subthreshold episodes.
