About this screener — evidence, source

Ten questions developed in a six-country WHO study to detect hazardous and harmful drinking and possible dependence over the past year. Questions 1–3 cover consumption, 4–6 dependence symptoms and 7–10 alcohol-related harm. The total (0–40) places the person in one of four WHO risk zones, each linked to a level of intervention from alcohol education to specialist referral.

Evidence · validated

In the six-country WHO development samples a cut-off of 8 gave sensitivities for various indices of problem drinking generally in the mid-0.90s, with specificities averaging in the 0.80s (Saunders et al., 1993). Many later studies in primary care and other settings confirm good sensitivity and acceptable specificity for ICD-10 alcohol use disorders at a cut-off of 8 (WHO manual, 2001). Sensitivity may be lower in people over 65.
